While you may want to believe that hospital’s have our best interest in mind when it comes to continuity of care and allowing access to your records, the reality is that some will indeed not follow their legal obligation to provide the records that are obligated to you by law.
The purpose of this document is to teach you HOW to request your records, what to ask for and then what to look for when you get them. There are some hospitals who rely on patient’s to NOT know what is legally obligated to them in a record set. If you don’t know what to ask or look for, it very likely won’t be given to you.
